Life & Disability Insurance
LIFE INSURANCE
Life Insurance provides additional financial protection for you and your family in the event of your death. Employees who work 32 hours or more per week are eligible for this benefit. The amount of the insurance is equal to your annual base pay rounded to the next even $1,000, but not more than $100,000. In compliance with Federal requirements, taxes are deducted from your paycheck for life insurance coverage equaling $50,000 or more.

SHORT-TERM DISABILITY
The Short-Term Disability program provides financial protection for you by paying a portion of your income when you are Disabled. The benefit is in the amount of 66 2/3% of your salary. This benefit begins after your fifth day of disability.
LONG-TERM DISABILITY
The Long-Term Disability program provides financial protection for you by paying a portion of your income when you are Disabled. The benefit is in the amount of 66 2/3% of your salary. This benefit begins after your 90th day of disability.
